Applicants must be current undergraduates with more than 60 credit hours by the end of the 2018 Fall semester and a minimum 3.0 overall grade point average. Applicants must be enrolled for the Fall 2018 Semester. It is not required that the student be a member of Mortar Board organization to receive a scholarship. Applicants will be selected for one of three $1,000 scholarships. The scholarships are open to ALL undergraduates attending OSU who meet the above criteria.
The application is due to the Mortar Board Email (osumortarboard@gmail.com) by Friday, March 2nd, 2018 at 4:30 p.m. For full consideration, please type the entire application. Recipients will be invited to attend Mortar Board’s initiation ceremony on Sunday, April 8th at 1:00 p.m. to be recognized.

1. Using ONLY the headings provided below , please list your activities and honors. DO NOT include
your name in this section of the application. Include activities and awards that are community as well as college related. **Please do not refer to specific living group names, i.e. “Wentz Hall”**
- A. Honors and Awards –List award/honor name, organization it was awarded through, and why it was awarded (Dean’s/President’s Honor Roll does not count)
- B. Community and University Activities
- C. Community Service and Research – Include event or organization name, how often you volunteered (one time, weekly, monthly, etc.), and the number of hours that you volunteered.
Example: Into the Streets One-time event 6 hours - D. Work Experience – Include title and organization, duration of employment, and hours worked per week.
Example : Cashier at Old Navy 6 months 20 hours per week - E. Scholarships – Indicate name of scholarship, year it was awarded, total monetary value, and how many years it is applicable.
- F. Other Activities – Please be very descriptive about the nature of the activity, its time commitment, and the duration of the activity.
2. Briefly describe your vocational plans upon graduation.
3. Briefly describe why you feel you should receive one of the scholarships. Include any special
circumstances of which you would like the scholarship committee to be aware.
