Photo/Video Model Release Form
Per Oklahoma’s statute protecting the right of publicity (12 O.S. § 1449), use this form to get permission to use a person's name, voice, signature, photograph, or likeness for purposes of advertising, selling, or soliciting purchases of products, merchandise, goods or services.
Copyright Release
This document provides a copyright release for photos on the Kitchen Sink Photo Database and grants permission for any person to reproduce, copy, print or enlarge any photograph on the site for any non-commercial use. However, please include a photo credit when using DASNR's photos (i.e., Photo Courtesy of Oklahoma State University).
